Diabetes Mellitus, commonly known as diabetes, is a chronic condition associated with abnormally high levels of glucose in the blood. The increasing popularity of smartphones with sensing capability is allowing researchers to design and develop mobile applications. The main problems that exist in our country in the diagnosis, treatment, and advice in healthcare services for diabetic patients include the following: First, there is no effective monitoring and close watch (follow-up visit) of the patient. Second, there is no on-time response for patients because patients are expected to wait until there turn to reach. Third, patient card's that holds patient history or profile might be lost during a follow-up visit, work overloads of health workers and so on. For this study, a cross-sectional study was conducted in different private and governmental hospitals, health centers and clinics. A purposive sampling technique is used to select domain experts for knowledge acquisition and different documents such as manual, guidelines, literature reviews, Internets consulted to acquire explicit knowledge.
