The WHO defines an "Adverse drug reaction as any response to a drug which is noxious and unintended and which occurs or doses normally used in the man of prophylaxis diagnosis or therapy of disease or for the modification of physiologic function". The American Society of Health-System Pharmacists (ASHP) defines significant ADRs as an unexpected, unintended, undesired, or excessive response to a drug that includes 1.the Requires discontinuing the drug 2. Requires changing the drug therapy 3. Requires modifying the dose 4. Necessitates admission to the hospital 5. Prolongs stay in a health care facility 6. Necessitates supportive treatment 7. Significantly complicates diagnosis 8. Negatively affects prognosis or results in temporary or permanent harm, disability or death.
