Surveying the medical records of this population makes it possible to identify their characteristics in order to subsidise interventions, facilitating patient care, promoting means of communication between health professionals and ensuring continuity of care and a sense of multi-professionalism, with the aim of contributing to advances in both the work process of health professionals and the evaluation process of basic health services.
