Clinical pathway is a structured, standardized, multidisplinary and interdisplinary plans of evidence based clinical practice/care, developed by specific health team member, and designed to support the implementation of clinical guidelines and protocols for specific groups of patients with a particular diagnosis in specific time by using clinical and non-clinical resources. Clinical pathway for the patients with chronic renal failure undergoing hemodialysis is very essential to provide high quality care, decrease potential complication, improve outcome, reduce unnecessary delays in care, decrease cost, decrease morbidity and mortality rate for these patients.
