Anterior tibial spine fracture or massive spine fracture in the pediatric population is rare. There are many surgical techniques, but they have the same therapeutic objective. Hence the interest of this work, which aims to describe the surgical technique and report the anatomical and functional results through a retrospective study of 17 cases of spine fractures, collected in the pediatric orthopedic department of the Béchir Hamza Children's Hospital in Tunis.The most frequent traumatic mechanism was flexion with a fall and anteroposterior femoral impact on a fixed tibia (bicycle falls) in 8 cases. According to the Meyers classification, 30% were type II, 41% type III and 29% type IV. A closed diaphyseal fracture of the femur associated with a fracture of the medial condyle and the homolateral fibula. The Lysholm functional score at 2 months after deplating was 84 on average (extremes: 72 and 92). At the last follow-up, it was 98.2 (range: 80 and 100).
