Most of the Endodontic diseases of pulp and periradicular origin are treatable by conventional procedures. Wheareas there are number of condition with problem of access, healing and nonresponsive lesions require retreatment which demands innovation. Carr's often quoted statement "you cannot treat what you cannot see" is most applicable here. Microsurgical endodontic treatment depends on the accuracy of diagnosis, appropriate case selection, the quality of surgical skills, and the application of the most appropriate haemostatic agents and biomaterials for its success.
