Refractive photokeratectomy has long been the reference technique because of its simplicity and effectiveness. It was quickly supplanted by LASIK, except in the case of weak myopia, because of slightly delayed visual recovery and postoperative pain. It is once again very popular because of its long-term safety, particularly because it does not cause corneal ectasia at a distance. The use of pharmacological agents such as mitomycin has allowed to enlarge its indications by reducing the haze phenomenon in medium ametropias. The preoperative consultation and workup to select patients eligible for refractive surgery is the most important step in the success of the surgery, and corneal topography is the most important examination in detecting frustrated keratoconus or corneas at risk. Although most patients who have had surgery recommend it to their loved ones, refractive surgeons should always be cautious and not hesitate to refuse to operate on patients at risk for complications.
