The adaptive survey questionnaire from Castillo (2017) was distributed to the grades 5 and 6 of San Rafael Integrated School teachers and students. With the tabulation and analysis of data, it revealed that the level of ICT integration is rarely practiced. Moreover, in local school ICT practice also revealed that the respondents differ significantly in terms of sex and role in school. However, it didn't show any differences in terms of grade level.
