Spermatic cord torsion of an undescended testis is a rare but serious surgical emergency. Material and methods: We included 21 cases of torsion of an undescended testicle, which were operated on in the pediatric surgery department of CHU Fatouma Bourguiba Monastir. This study covers a 16-year period from January 2005 to July 2020.Results: The mean age was 2 years. Sixty-two percent (62%) of patients were followed for testicular ectopia and 42% had cerebral palsy. Inguinal swelling was the most frequent reason for consultation ( 95% ). Clinical examination revealed an irreducible painful hard inguinal swelling with an empty homolateral bursa in all patients. Local inflammatory signs were present in 43% of cases. The left side was affected in 76% of cases. The diagnosis was confirmed by surgical exploration in all patients. Orchidopexy was performed in all children.
