It connects Virginia State Route 259 south of Mathias in Hardy County, West Virginia to Virginia State Route 259 at High View in Hampshire County, West Virginia. WV 259 runs North from the Virginia/West Virginia state line in southern Hardy County through the communities of Mathias, Lost City, Lost River, and on to Baker. At Baker, WV 259 runs concurrent with West Virginia Route 55 North to Wardensville. From Wardensville, WV 259 continues alone North through the communities of Yellow Spring, Lehew, and High View. At High View is the Virginia/West Virginia state line in eastern Hampshire County. Here it becomes Virginia State Route 259 and is also known as Carper's Pike. It continues in Virginia and terminates at U.S. Route 50 near Gore, VA in Frederick County.
