Clinical pharmacy has been defined as those services provided by pharmacists in an attempt to promote rational drug therapy that is safe, appropriate and cost-effective. It is important to appreciate that clinical pharmacy can be practised both in community and hospital pharmacy environments, including teaching hospitals, private hospitals, regional/district hospitals and nursing home.
