Intumescent white cataracts pose challenges in every step of cataract surgery. Capsulorhexis is the most challenging step followed by nucleus management. The capsulorhexis is prone to anterior capsular radial tear extension or also known as the Argentinean flag sign. The nucleus management by phacoemulsification becomes further risky in the presence of the Argentinean flag sign. In this article, we will discuss how to achieve intact capsulorhexis in maximum numbers of cases, if intact capsulorhexis is not achieved then how to proceed for safe nucleus management.
