Patients with chronic kidney disease (CKD) often have alterations in their pharmacokinetic and pharmacodynamic response. They constitute a population at high-risk for adverse drug reactions and drug-drug interactions. Drug dosing in these patients is a very challenging task. This book, therefore, implements a dosing service by a clinical pharmacist and evaluates its outcomes. It includes an introduction to CKD and the steps for suitable drugs' use and dosing, and then it includes methods, outcomes, discussion and conclusions related to the service. Part of the study was to utilize a collection of reliable and up-to-date references that are commonly used globally to develop a pocket size handbook for dosing recommendations related to renal impairment - if present- and removal by dialysis.
